Procedures for Requesting Short Term Appointment Effective 1997/98 all new and reappointments will be processed during Spring Quarter and will be due by April 15. To request a Short Term appointment, please have the sponsoring faculty member fill out and sign appointment form available in Sherry's shared folder. In addition to the form, you will need the following: 1. Updated CV 2. Course number and quarter/s (AAC approval required) 3. Duration of salary (if different from appointment) 4. Account number/s to charge salary for research 5. I-9 (Immigration Status) form for salaried employee 6. Patent and Copyright Agreement Some things that may not be included on the cv that are required are: a. Social Security Number b. Birthplace c. Birthdate d. Citizenship e. Visa Status The sponsoring faculty must state the intended role expected of the person and his/her reason why this person is important to the laboratory. Please include account numbers with salary as well as a breakdown of percentage of time if using multiple accounts. This appointment is processed in 3 stages and can take 4 weeks before a confirmation letter is generated from the Provost office, so please allow sufficient time. The Provost Office enters the appointment into their Faculty Affairs Academic Secretaries (FAAS) database that is used by the Registrars office to verify the appointment status and issue the candidate a courtesy photo ID card.** You do not have to wait until you receive a confirmation letter from the Provost before you call the ID card office. **Eligible short term appointments in your lab (lecturers, visiting faculty, consulting faculty) can obtain a courtesy photo ID card which covers the whole term of their appointment (up to one year), rather than the associate cards that do not have a photo and must be renewed every 6 months. Once the appointment is in FAAS, the person should telephone Tiffany at the Registrar's Office (498-2273) to confirm their FAAS listing before they go to pick up their card. If all is a "go" they can then receive a photo card. Tiffany is located at the Registrar's Office, Old Union, Room 140A. The FAAS database requires that every record have a Social Security number (SSN). However, if the visitor will not be obtaining a SSN, he/she will not appear in FAAS and their sponsor will have to call the Provost office and have them fax or e-mail the appointment status to the Registrars office in order to obtain a courtesy photo ID card. University ID cards are issued to those who have a salaried appointment at 50% or higher. 11/97
